If you are installing via the disc, this is a more or less common problem. The disc has to be in very good shape. Scratches or dirt can make the install impossible... Try cleaning the disc and do it again or read the forums to learn how to install with your pc over network.
Or just install via IDE to your ATA drive....Connect the HDD to your PC and install with WINHIIP.....
Unless this has been reported elsewhere for your game, it's your disc that's the problem.
(notably gradius III and IV US will always fail to install no matter how clean your disc is)
Suggest ripping it with dvd decryptor and using hdl_dumb to install. hopefully it will read without errors. if not, then get (or rent) another disc.
maybe laser is worn out, like my ps2 can't install games eventhough is new (just bought it, it really wont read the damn dvd game.) only option is to install by pc...
