1/2 - Possibly, however on the PS2 Slims when the IDE hack first came out the majority of people had that issue pop-up, I however, never had that issue and my games installed and ran perfectly fine.

Yes. An insufficient or unstable power supply may result in the disk not functioning correctly, and may result in errors occurring (I/O errors).
If you want to reduce the number of possible reasons for this fault... you could borrow the PS2 of someone else to test your NA and HDD to ensure that your PS2 isn't at fault here (Although it should be your NA causing this).
