Independence Compiler will get the format correct and includes a fair few elf files to get you started. The file can be downloaded Here (You may need to login).

Basically you type in the ID Code of a ps1 cd (Or click GET ID if you have it in the drive). You then select the menu you want, and the apps you want.

Type in IP addresses if you need them, specify a dummy file size if you need one, and lastly select the format.

It can either just extract all of the program files you've selected plug their config, and the TITLE.DB, OR it can extract it in Independence CD format. Instructions for burning it are included if you need more help just ask on the support forum over there. (Exploit Sender questions should also go over there!)
