Sorry just to check the obvious first:
You are not trying to run discless?
You are running from the disc icon not app_home/PS3_GAME/ ?
BD-ROM Emulator set the Standard?
Have you tried BD-Mirror?
Meanwhile I will have a closer look at the EBOOT.ELF to see if I can see anything I missed.

Just tried BD-Mirror, no luck. Tried BD Emulator on Standard and Hermes, no go. Tried disc and discless, not working.
One thing I did try and had a bit of success with was using your eboot and letting MM change the original .SFO to 3.55, the title screen loaded but then froze my ps3.

Sorry yes your are quite right I messed up the param.sfo changed the version to 03.400 instead of 03.4000
. But yes Multiman will fix it correctly.
Works on 4.12 with the following settings: BD Mirror off and Standard BD-EMU.
I can not see anything else in the EBOOT.BIN that needs fixing, will look more later.
Please can you do one last test:
Delete the game data
BD Mirror off and Standard BD-EMU and let MM fix the PARAM.SFO.

I'm beginning to think there are possible checks within appldr or some other files besides the game files that are making checks on these eboots.
It is strange that we can fix for 3.55 but they won't work on 3.55 yet the same files work on 4.21
Pre 4.21 files are still 3.55 fixable so something besides new offsets are being checked, quite possibly at a FW level.
Could be something simple like a version check of any libs called by the eboot?

Yeah, that's what I'm talking about. It seems more and more are becoming unfixable so its better just to update.
