you need just 4 connections to program the chip as you have described...
OSC1 on blitz goes to pin 27 on SX
OSC2 on blitz goes to pin 26 on SX
Vdd on blitz goes to pin 2 on SX
Vss on blitz goes to pin 4 on SX
you also need to connect pin 28 to pin 2 (mclr to Vdd) on the SX
I have a ZIF socket on a prototype board that i use to program blank chips, wired as above.
For in circuit programming, i always take the +v and gnd wires that power the chip and desolder them from the ps2 completely and just route them to the programmer/external supply.
Osc 1 and 2 just have flying leads which I solder on and in some cases leave on even when the ps2 is fully modded. Each of these 4 wires are around 30cm in length while programming I would say.
