I wonder if others have an explanation to some issues I have seen the Last Game do on my PS3.
Sometimes it will show the game icon of another game and the title will be from the last game I played.
Sometimes it will keep showing a game I played a while back, but not the very last game I did play.
Anyone know why this occurs? Or am I the only one?

Its buggy for me as well, it doesnt always copy the data to my XMB, seems only a handful of games are compatible with it.

The name of the game is usually wrong, you have to look the preview on the icon, if there's an AC: Broterhood preview and the name is Motorstorm, then the game is AC: Broterhood.

here's how it works,the reason the game doesn't always change as the BD Mirror function does not work with the last game method, game name not updating is because of PS3's cache method (as I believe either a system restart or running the app will update the cache)

In fact you are partly wrong. The problem exists even without the BD Mirror method, though that may add some 'wrinkles' to the problem.
I myself never use that method, but I too often get the mismatch between 'last game' title displayed and the icon picture. And for me too it is always the picture that is correct, while the title usually shows the game used previously to the latest one.
Dean has explained that this is due to how the XMB works, using a title string cached somehow so that MM's code can't modify it directly. It can only modify the icon picture file directly, which is why this is always correct. The title string has to be modified by XMB itself after updating that cache, which usually happens some time after running the game, though it's sometimes not correctly updated until the next boot session.

Exactly.
Without knowing a lot more about the internal workings of the Sony XMB, we simply have no way to interfere with how it caches things like that title. And to fix this string update we would need a way for MM to forcibly change the copy of that string in the cache of the XMB.
So for the time being we simply have to accept that it works this way, with the icon picture updated directly, but the title string being delayed until the XMB 'wants' to update its cache.

What about when it still shows the icon and title of a game I have played a while back, and even though I select new games from multiman, that old game's title and icon still show? That was my second report above about that it also does that for some odd reason.
And I tried loading it, and it still loads that old game, which has been a few games ago. And yes, the old game that it shows was a game that I needed to activate the BD feature. Plus the new games I've been playing also requires the BD feature to be enabled. Hmm? Perhaps there is an issue with that?
Please let me know if you have any suggestions? I also have the BDROM reset, and I do run it to see if it will help, but it doesn't.
Update:
I loaded a non-BD game, and now I see that the Last Game app representing the newest game I just loaded. So there is something definitely wrong when you select two BD games back to back. It will continue to show the last BD game that required it, and the next BD games it will never show them in the Last Game app--not until you switch to a game that doesn't require the BDROM feature. But now it shows the icon of the new game just loaded, but not the title--the title is still that old BD game that I haven't played for days..--weird!!
