After numerous hexes and managers found this to be solution...
1. Boot with Hermes 4b hex loaded. (assuming this is what you installed the game files with)
2. Install permissions fix package from USB and run it. (search for it in another thread- it is post #195)
3. Install latest PL3 hex (one with no unauth syscall)
4. boot and launch game with any BD in drive
5. Will install game data and plays fine!! (Did not install 1.01 update)

The new version apparently patches the sfo for you so you don't need to patch it.
Also, what is the ID of your MOH?
I could only get BLUS30436 (US) working.
This is how I got it working:-
Installed the patch from USB (via Install Packages option) first.
FTP'd the game over
Put a game in the drive.
Started Gaia, selected patch mode, selected the game (it went back to XMB), started via the disc icon, not the app path)
Game started, installed files and then started the game after.
If I ran without patch mode enabled, it just froze on 0% when installing files.

Which patch mode are you talking about? Just the >3.41 patch or both?
