Did you tried with Game Disk without jail break in service mode. Sound thingi means you are in service mode. If you fail, bring ps3 out of the service mode. go to recovery mode and update the same 3.41 (modified) FW. Then go to service mode again and give a try.
Its happened to me also some times. What I understand so far for my case is related to storage regions. I will post back once I will have some solid reason.
Best is to use "UP" Game Disk. as its shows message box saying not enough space on hard disk. And you can safely exit from game. I tried one more thing. Instead of game disk i put orginal movie DVD. ( i dont have blue ray disk ). RSOD goes for a while and then comes back. may be BDVD/DVD software trying to access HD and fails. But this give me one more option to try which i will. I will replace the BDVD/DVD self with ftp ( using freeflash and ftp ). as far as dev_flash is concern I checked all the files with cksum ( ftped to my PC ) there is no problem. One time I replaced all dev_flash with Hermes CFW ( just and expriement) there is no change expect XMB option of Intall pkg. Same I did with ReBug dev_flash. ( system refuse to start - I reflash the 3.41M ). I tried Acid CFW - result in some fancy XMB but no fruitfull result. This weekend I will do some more work. ( In Gulf weekend is thursday & Friday).

i have been fix rsod once time in recovery mode option3...when the ps3 firmware restored...the rsod was disappear... then the screen shows found HDMI input and ask me wanna change to 1080p mode...i selected yes...ps3 still ok...finally, screen shows ps3 system needs fix...after it fixed...rsod come back again
Using dev_blind code I am able to mount dev_hdd0. But still some thing missing. I tried to copy movie (failed). tried to run UP game using blueray disk ,it found the hdd0 but hang after first button press. hdd0 is showing files and directories.
Nothing worked more me either , i have tried everything you guys have.
looks like we just have to wait for a payload to be released the allows us to use a flashing app or even an app that automatically repairs errors. couple things i have noticed that its different from you all
i can enter into service mode and recover mode
my update log from downgrade says everything is a success
i plugged an ethernet cable in the back of it and it still acquires an ip address
if i hold down the PS button it gives me to options shut off controller or system and it only displays the time
i can even ping my ps3 and it responds
Question are they are any custom firmwares that install and activate a ftp server right away on the ps3
Another Question IF some of you guys claim to have the RSOD how can you activate FTP?
I am looking for the possibility to add OpenBackupManager/GaiaManager EBOOT part of FW. Launch it automatically. So far i dont have success. I replaced onicore_child.self with OBM EBOOT but there is no change. Old Dev must have some knowledge about autoexecution. I will try with other self also. By the way the PS3 I am working on is Slim. (NOR + VFLASH).
i have no idea what that means i am new to the whole ps3 scene , i use to be an avid xbox360 owner so i am having to learn stuff..

Just do a test. Connect external DVD with some movie disk inside. Similarly AVCHD on USB and DVD. There are auto lunch senarios which I am no aware of. Do some test and share the positive results.
Add: upnp servers might have some role. Need to be checked over working PS3 first.
I will try AVCHD via USB on my working PS3 (Phat).
