18f4550 help please.

I received my pic 18f4550 and the parts i need to build psgroove, its a 40 pin chip and i am going to program it with an art2003. Which diagram do i follow for the schematic and which bootloader hex do i need to program it with so i can program over usb. Most importantly which pin will it be to put it into bootloader mode when i tie it to ground? there are a ton of schematics for the 28 pin and like 2 vague ones for 40pin ones. There is one that says tie pin b5 to ground if using lvp... when do i tie it? before programming with art or in the final schematic?
