1) What Model is your PS3 (Phat, Slim Super Slim?) A CECHXX-xxxx will be enough to answer that you DO NOT need to give the S/N in front of it!
2) What are you currently running? OFW? CFW? Whats the Version Number? If you are on an OFW greater the 3.55 You'll have to be able to downgrade your PS3 back to OFW3.55 (OR LOWER!!) to do so.
3)Do you have "★ Instal Package Files" and "★ /app_home/PS3_GAME" in your XMB "Game" Menu?
If you don't its a fairly good sign that you DON'T have a CFW installed.
In the case of the "QA Toggle Flag.pkg" you download it from your PC, copy it to the root of your USB Stick. (i.e. NOT INSIDE A FOLDER!). Insert said USB Stick into your furthest right USB Port (i.e. the One closest to the BlueRay Drive). Then scroll to "Game" and down to "★ Instal Package Files" where you should be presented with all the *.PKG Files you have copied to your USB Stick. Choose the "QA Toggle" and it should install. You can test this out by doing the Button combo from the "Settings" Section, to verify your install.
Otherwise assuming that your looking for a way into the PSN/SEN again you have Two choices that I know of currently the Habib 4.46 CFW, and Rogeros 4.46 CFW. I've been using Habibs CFW for a couple of Weeks now and its IMHO as good as Rogeros 4.41 was. So I doubt it matters which One you use. They both do pretty much the same thing.
You Download your new CFW (Whatever it maybe...) copy it to your USB Stick in x:\PS3\UPDATE\
And don't forget to rename the File to PS3UPDAT.PUP VERY IMPORTANT!
Insert you USB Stick back into your PS3 (again furthest right), CLEAR your BlueRay Drive of any Discs!!, and go to "Settings" -> "System Update" -> "Via Storage Media" which should then allow you to update your PS3.
That is assuming of course that you actually can UPDATE your PS3 to begin with. If your on an OFW and its higher then 3.55 you are half way to being SOL, If you also have a 25xx, (Greater chance). 3xxx or 4xxxx (NO CHANCE!) then you are almost fully SOL! If you have an older Phat then your only option to downgrade is to do some soldering. As there is no real solderless NAND Flasher available.
Otherwise like I said your only half screwed, and you'll have to invest in a E3 Flasher.

Place the *.PKG File on a USB Stick, and select "Game" -> "★ Instal Package Files" (I forgot if you have to point to the USB or not), find your *.pkg File and then install it... Simples....
As for jumping to a new Firmware you do that in much the same way as you would if you had to do an off-line update with the USB Stick.
Take a Firmware (for example the latest Rogero 4.46... http://psx-scene.com/forums/content/...released-3545/) You just download the File and rename it from RB446DKIPF.PUP to UPDATE.PUP and place it on your USB Stick...

Update data download procedure
Create a folder on the storage media in which to save the update data. Using a PC, create a folder named “PS3” on the storage media or USB device. Within the PS3 folder, create a folder named "UPDATE".
USB Storage -- PS3 -- UPDATE
(i.e. x:\PS3\UPDATE\) x= Where ever your USB Stick turns up....
Download the update data and save it in the "UPDATE" folder created in step 1.
Be sure to save the update data as noted below. If the data is not saved in the correct way, the PS3™ system will not recognize the update data. The folder name must be in all uppercase letters.
Location: Save in the “PS3” folder > “UPDATE” folder
File name: Save as file name “PS3UPDAT.PUP”.
ALSO BE SURE TO REMOVE ANY DISC YOU MAY HAVE IN YOUR PS3 PRIOR TO UPDATING IT!
