    Have you tried recovery mode?

    Start with the PS3 off (in standby mode)
    Hold the power button and the PS3 starts up. Keep holding the button till it shuts itself down again, then let go of the power button.

    Press and hold the power button again, and the the Ps3 with start up and give 1 beep............... then 1 beep........... then 2 quick beeps. LET GO OF THE BUTTON NOW.

    The PS3 will start up with the recovery menu which will allow you to re-flash the OFW.

    no matter on recovery mode or via XMB

    your USB Drive should be formatted to FAT32
    and make sure your setting is correct updating via USB Drive ;
