So I have a pal version of Destroy all humans, but my Ps2 is NTSC. Is there any way I could patch an ISO of the game to run as NTSC and then just load it up with ESR?

Shouldn't you just patch the game like normal and boot it up from ESR?
Or can't your TV support PAL signals?
I don't know if PAL to NTSC is any different to NTSC to PAL but, I have been running NTSC backups of games like Xenosaga Episode 1 through ESR on a PAL console with no problems. I didn't have to convert it to PAL or anything.
I know there is some program out there that people have used to convert games from one format to another like PAL to NTSC and so on. I don't know what the program is called. You could try googling 'PAL to NTSC converter'.

All consoles can output video signals of both PAL and NTSC.
The only limitation would be the regional lockout.
But if you are using ESR, that should not be a issue.
If you need to get the game to output a NTSC signal instead, you might want to use GSM (Available on the forums here).
