Update: I managed to find the answer to my confusion by myself, it's the ESR GUI that's 384 kb! Doh! 
Im slowly getting the hang about this FMCB and ESR, but I still have a few questions im trying to figure out.
In both the noobie package and the FMCB installer package the ESR.ELF in the install folder is 384 kb.
But ESR beta r9b dvdv direct.elf from the latest ESR package is only 28 kb!
Have I misunderstood something here, or shouldnt the "dvdv direct" be renamed to ESR.ELF and put in the install folder?
Just to make sure I did everything right, I'll give a rundown of my install process to see if anyone can spot anything wrong.
Latest FREE_MCBOOT.ELF and the INSTALL folder at the root of my usb stick.
In install, I have put the latest uLaunch, renamed to BOOT.ELF, The original ESR.ELF that came with the package (384kb)
And in the INJECT folder I put the proper file for my region (Europe) and renamed it to DVDELF.BIN
And that's it! I dont need/want sms, hdloader etc. Anything wrong with the above? Anything missing?
The main question, and the reason for the ost is still the difference with the ESR.ELF file, and the files from the ESR public release package.

That is because it allows all the different operating modes of all the other ESR elfs, plus some more that is unique to the GUI version (to deal with problematic games). The disadvantage is that you must specify options manually at each game disc boot.

That is because it only allows a single one of the many boot option combinations. This is admittedly the one most useful for game booting, but there are some things it can't handle.

That is what many people do, including myself, reserving the GUI version for manual launch from uLE only for those occasions when it is called for.

No, that sounds fine to me. I assume of course that you also kept the mandatory stuff in the "INSTALL" folder (inside subfolders "FMCB_CFG" and "MODULES").
The one objection I have is that I'm surprised at anyone not wanting SMS, but that is entirely up to you in any case. For me it has become my main method of watching both TV series episodes and movies, by streaming the media files over LAN from my computers to the Samba client of SMS. But if you don't want that kind of thing, that's your choice...
The main question, and the reason for the ost is still the difference with the ESR.ELF file, and the files from the ESR public release package.
That issue is simply due to the fact that the guy who put together the noobie package wanted to provide a way to support all ESR usage modes with a single file, and the GUI version is the only one which can do that.
For example, when you boot with an ESR disc in tray you can then choose to have the GUI launch it as a bootable disc, or to have the GUI just activate ESR drivers and launch uLE instead, so you can use the uLE FileBrowser to access the disc contents. You do gain some flexibility that way, at cost of having to specify what to do at each ESR disc boot.
Personally I always boot into uLE however, having turned off the 'FastBoot' in FMCB configuation, so if I want to browse an ESR disc I simply start the 'mcard' ESR elf from uLE. And when I want to directly boot an ESR disc I do it by at boot pressing a button configured in FMCB to use the FastBoot method (regardless of the other FastBoot option being off), and this then launches the 'dvdv direct' ESR elf (stored as "BOOT/ESR.ELF" like you wanted).
Since I normally boot to uLE, I get all the flexibility I need from it, so when I do use FMCB FastBoot I want the simplest and fastest boot possible, which I get with the 'dvdv direct' ESR elf.
There really isn't any single 'right way' to configure FMCB but many different, according to taste, so do as you please and see where that gets you. Perhaps you'll want to change things around again later, and that's OK too. FMCB was created to give the user maximum freedom, not to impose any limitations.

Thanks dlanor, that made everything crystal clear.
And I dont want to use sms since I use my "normal" dvd player for dvd's. Saves the ps2 laser and such...
