Does Region Matter? –
Does region matter with ESR? Or does it not matter at all? Just a curious thing I wanna know.
It does not matter. If you look at the "ESR compatibility thread" you can see that everyone is using games from diff regions then their console.
But a PAL game on an NTSC console/TV will still be distorted, correct...?
It will be much more than distorted. It will be black and white and rolling from top to bottom. You can patch some PAL games to NTSC ( and vice versa) with the right tools though.

My experience learns that patching from PAL to NTSC is easier than the other way around, in the sense that the patching is almost the same, but NTSC -> PAL doesn't do the sound any good, nor the video.., No imports for me
That would be a multi-system TV. I have such a TV,(Akai brand) but that feature is very rarely ever marketed in the US. So you would have to find out by visiting the manufacturers website, or by trial and error. Not even all new HDTVs have this feature, though most do.
