"Please insert PS2 Disc" with confirmed FIFA '08
Try swapping the disc at various points it has to done in a way that ps2 still thinks it's an original try doing whilst it's playing a video file or when the game is less active ie: not in-game or playing music, loading multiple textures "yes even main menu's read alot of data from the disc constantly"
to confirm you've swapped the disc properly try playing your patched backup if it plays fine then go to the net config to load NETGUI_EU.ELF or NETGUI_60.ELF. It's all about timing.

2x is more than likekly too slow try another reburn at 8x.
Also you don't need to patch the image with Momento Disc Patcher or ESR that's only for running homebrew disc's or backups through the ESR or Momento software which your are not doing here, what your doing here is a simple disc swap, what software did you use to replace the files? do the LBA addresses remain the same as the original disc if you compare it in IsoBuster?
I have the same problem , tried installing FMCB using Swap trick ( FIFA 08 )
replaced the files , swapped at the tutorial video , main menu , in the online mode menu but still failed. All what i get is the RSOD . And i think i did everything correctly
You are opening the disc drive for swapping by using a slide card right, and not by pressing eject?
Back in February I did this hack on my friend's FAT PS2 SCPH-3003 using FIFA 2004. I tried it on my Slim, but when running the .elf it would say insert a real disc, however the fake disc could be played perfectly. I guess the Slim's have extra security. I installed the hack to my mem card and it worked fine in my Slim though
I swapped at the menu IIRC.
So u mean it wont work for me ?
Are you swapping discs by forcing the drive open using a slide card or by normally pressing the eject button?

Ahh I see. There's your problem then, as I wrote (I guess you were referring to my post about the Slim having extra security). AFAIK Agent Under Fire 007 is the only game that Slim's can be hacked with using the swap trick. Otherwise if you can get your hands on a fat PS2 or if a friend of your's has one, there is that option.
