Are you sure the quality of your media is ok ? Plus is your laser in a good condition, right ? I doubt anyone could give you the proper answer(s) regarding your problem. These two problems which I mentioned above are the most cammon, try another media, written at 4x (Verbatim used to work) and try to clean/adjust the laser. You did not mention what PS2 version you have, older ones have picky lasers. And also forgot to tell what DVD burner you use, they are quite different in quality, so with so little info I cannot suggest you more.

    regards, Stephen

Well, I suggest you should try some other media at faster speeds. Go to www.firmware.fr.st to check what is the latest firmware for your burner, and upgrade it if needed. Original games have much better reflectivity than written ones, especially if the burner/media does not match, the result is questionable. I personally installed a brand new laser into a V4 a few weeks ago, Verbatim DVD+R works fine but not that many other media boots for the first time. The usual brands like Ritek even do not work at all... So 1.5 years is a long time in the PS2 world, unfortunatelly Sony lasers are far from the best ones...

Browsing this forum you may read that several guys with newer machines than yours also having reading problems. Unfortunatelly there are only two methods to test what is your problem:

    - trying some more media (TY, Fuji, minus media)
    - checking your existing media with a PI/PO testing program

As Sony DVD-R's are usually rebranded LiteOn's, there are several LiteOn drives that have writing quality problems...
