Could be because of your software or the firmware of your DVD burner.
    Owns:
    Playstation 2 V4, with Duo2se modchip and 400c Laser.

Definitely stick with DVDDecrypter. It's freeware and is a great app for backing up PS2 discs.

What's the media brand? Stick with Ritek/Ridata and you should be fine. As someone suggested, also check out if there is a newer version of the firmware on your drive.

As others suggested, use DVDDecrypter and use quality DVD media. Verbatim, Taiyo Yuden and Ritek G04 are the most reliable. Also if your PS2 is an earlier version, try to stick with DVD-R media (as opposed to DVD+R).

Given the way that Ritek's quality and consistancy has been going lately, I won't touch them with a 10 foot pole. Definitely look into a firmware update from rpc1.org though. Quality media makes a HUGE difference. You should be able to burn quality media (ie Taiyo Yuden, Mitsubishi Chemicals/Verbatim) at max speed with no problems.
