Yepp, it more sounds like a reading-problem, which is why I suggested you to use an other "brand"/Manufacturer.
But still, it is worth a try to make a fresh ISO, patch it with Toxic-DL-Patcher and ESR-Patcher then.

Don't use ESR 9b! Use ESR 9a. I've got the same problem and the game wasn't loading at all! Just a black screen after ESR flashed all the colors.
I don't know why but I've got this error on both PS2 (a PAL SCPH 54000 and 90000)

Then I switched to ESR 9a and the game play just fine after patching them correctly (Toxic + ESR)
Ok, I tried that........ didn't work....... 9a doesn't even read the disc, with 9b i atlest get to the colour screen....... however, I get a feeling it might be my dvd writer, so i'm gonna try a new one tonight, I'll let you know what happens......!!!
One question though........ wopuld patching the .ISO with the Toxic patcher and then the esr patcher realy make a difference......!?!?

It should make a difference, because the Toxic patcher looks at the contents of the filesystems, so if you use the ESR patch first then its modification of the UDF filesystem probably will confuse the Toxic patcher. I'm not sure what the detailed results of that would be, but it is obviously safer to use the programs the way they are intended.

Ok, this is where we are now:
Patched the .ISo with Toxic, then with ESR patcher, wrote it to DL dvd using a DL dvd writer at x2, tried loading it with the 9b version...... it loaded to the Sony GOW2 menu page and frose, then i tried loading it with the 9a version, and NOTHING happened.........!!!
Any other advice guys........??? i'm realy running out of ideas here.......!!!!!
Which type of dvds did you used for backing up your game?
Manufacturer/Brand? Staples/Spindle/10th-Box? Speed printed on it?

(I fully agree that Verbatim is the prefered media, however there are none available atm)
Have you tried all FMCB modes? There are three different elf-files you can try. They should be in the same folder as the one you use. "Dvd direct.elf", "dvd direct off.elf" and "mccard.elf" Try these. Itīs just a guess. It could be worth a try.
