CFW on PS3 Fat Failing Super Megahard –
I have a US PS3 60GB fat version. I perviously used an e3 card to play backups with backup manager on firmware 3.41 (but did not instal cfw 3.41).
I upgraded to OFW 3.55, then installed kmeaw cfw 3.55. I checked the md5 hashes, and they were fine. Kmeaw would not install from xmb (version 3.55 already installed) so I installed it from system recovery. Everything installed fine. I then installed Gaia (2.07) manager and just to be safe I installed lvl-2 v9 even though I know it is unnecessary. I ran gaia, and ran an arbitrary backup. The game appears on the menu correctly, however, upon running it the screen will either
a) Go completely black and stay that way
b) Go completely black then after a few seconds return to the
I then tried running lv2 v9. I tested each of the three options then ran Gaia. All of them failed. One or two also failed to change the title of the disk.
Next I tried installing the latest multiman, and ran it with each of the three lv2 v9 patching options. All Three failed similarly.
I even tried lv2 v6. I tried lv2 (version one). I tried version one with Gaia 2.01. I tried Gaia 2.01 on it's own.
I now have tried installing 3.55 OFW, followed by wutangrza, then installed gohotz on top of it. I installed Geohotz from system menu, though after preparing for the update, the screen went black and it made a particularly interesting set of beeps then turned itself off. So it never really installed, but after that I was not bold enough to install it from recovery mode.
I have heard murmorings of eboots specifically for cfw 3.41, though I've never installed CFW on 3.41. This coubled with the particularly high volume of games I have and have tested I doubt this could be the sole issue.
I did recieve an error with each game which wasn't previously tested saying eboot.bin not found, fix permissions? I hit yes as the interwebs tells me to, and then the error message goes away forevrar. The interwebs tells me this is fine.

You do know some games blackscreen for a reason. What game were you trying and was this game modified to be played on 3.41?

They all had the same results. All of these worked on 3.41 (using E3 not CFW). None of them were modified for cfw 3.41 as I mentioned. I don't think it's a game specific issue.
You just described the exact experience I am having with my sons PS3. Not been touched til 3.55 (is CFW KMEAW) and he is having the exact same issue. I thought it was something to do with all these syscalls flying about, glad someone is also having the same problems (albeit would like to solve help solve them!)
Anyone able to find out why a virgin PS3 does not seem to want to play ball the same way my one will, although they are setup in exactly the same way...?
None of the games have modded eboots. They are all the same as mine (on my system) and all work perfectly. His refuses to work

Wow...sorry for your troubles. I did both my Slims with no problem at all. I would suggest...and this is JUST a suggestion....to revert your ps3 back to OFW 3.55 to start with. Then install Kmeaw CFW 3.55, then Gaia, Multiman, and Blackbox. Do NOT install the Lvl-2 v9...it is absolutely not needed.
Every game I have works with Kmeaw 3.55, I had to replace a few eboots that were modified (to work with dongle on 3.41)...but you do NOT have to replace the Param.sfo. Even previously not working games like Nail'd and Call of Duty: Modern Warefare 2 now work...absolutely no modifying of files...just different settings in Multiman.
Even Killzone 3 and Yakuza 3 work perfectly...just testing of course until my preorders arrive.

E3 or 3.41 custom firmware. It doesnt matter. If the game required a higher firmware to run then it required a custom eboot for 3.41. When you upgraded to 3.55 all those games with the 3.41 eboots wont work anymore. All i can say is if you downloaded the games, and downloaded them before jan 3.55 cfw, chances are they had modded eboots and you will need an original eboot. This is only for games that required a firmware higher then 3.41 to play
Also, don't forget to remove game data for any games you played on 3.41 (Game XMB - Game Data Utility - Delete (only deletes installed data, not game saves))
You dont have to start lv2.pkg everytime! Just use Multiman with BDEMU and it should work fine. Otherwise run lv2 permissions fix pkg which should fix it.
