is this a CD created with the instructions by you, or the CD that is available to download?
the early models of PS2, the ones with the PCMCIA HDD, are not supported (the first two models, i believe)
the reason is there are some firmware drivers or something that were not implemented on the systems way back then, which most PS2SDK projects (including SMS and uLaunchELF) need to work.
My own model of PS2 was a SCPH 50004, which is now broken. I can't test the CD myself though, but this method of creating CD's had always worked for me when it was working.
I believe that with earlier models, (30000 and earlier), the PS2 had dificulty reading from near the middle of a CDRW, and the work-around was to add a large blank file to the disc (about 20 megs worth) before adding the ELF file... you might like to try that, even if you do have a 50004. it might help, then again, it might not.)
