I've seen a japanese V8 fail like that without even having a modchip installed, so...
It is suspicious ...
What if the newer PS2 units have traps to cause failures when modchips are installed ?
I've noticed the bad install reports allmost doubled sincethe GH-022 mobo were instroduced.
It is VERY suspicious ...

I just thought a little more and figured out that if Sony is doing that kind of sabotage, they are aiming the small modders, as having random units to fail is like a technique to make people confused. As they are capable of making chips on several factories at the same time, they might have diferent versions of the same chips (PS2 internal chips) on machines of the same model and time.
This might end up being bullshit, but I am suspicious Sony is trying to make modders have monetary loss.
