Got a v5 motherboard here. When powered on there is no reset or eject light.. Ribbon cables are fine, reset/eject buttons are fine, all fuses test good. Power supply is good, 12 volts comming out. Motherboard is unmodded. Now i thought it might be the ba6664fm but im not sure since that controls the spindle..

I know it sounds stupid, but did you check wether the RESET cable is plugged in with the correct face towards the contacts ?
Check both at the motherboard and at the button.
Another solution would be to take another v5 board and with the power supply connected through some wires to compare the voltages at different voltage regulators (or fuses) and see where they don't match.
And why did you think of the BA6664? The BA5815FP is more likely to be blamed for this behaviour.

if it takes more than 2h to think about/fix a mobo i throw it away.
use the time to do another install and you have the mobo paid off (given your tariff is high enough
)
basically dick around for 1h then salvage any good parts...

or if you have extra time:
1. to check if any electrolyte caps went: heat the back of the mobo and the supply with a hairdryer untill its too hot to handle and immediately hook it up to see if it works.
2. or try swapping the syscon IC from another mobo (the square chip above the alt. bios space) (C807U)

I agree, you have helped so many other people I think you are entitled to have a sticky now and then. Off topic I know, sorry.

The buttons work and the console performs normally??
I'd say you have two non-functioning led's then. Have you tried a different switch and/or ribbon?
I had one console where the eject light didn't work, used a different switch on the same ribbon and voila! LED's last a long time but not forever.
