Should have mentioned that I'm installing in NTSC machines...
Yeah - just re-tested and all PAL PS1 games work 100% on my GH-041-04 PAL consoles.
Will test a NTSC PS1 game shortly and see if that causes an issue on my PAL consoles... if it does not then... well this is either a NTSC console problem or an install technique problem...
Ok tested NTSC games as well - no problems at all!! I think this comes down to either being an NTSC console issue or installers with poor layout...
example of my installs can be seen here (so far 3 have gone in and all work fine)

I don't think it's a NTSC console issue - as that's all I mod.
The install I did yesterday on the v12 was pretty much exactly like the v14 install I posted here (but without the LED):

Interesting... I had a suspicion that if people did not use the LED board that may cause a problem as I have always used the LED board...
But if you have tried both and also not had any problems then - it was either a bad batch or people really are doing something wrong when installing...
I tried installing it with the LED board and without both giving me the same problem.
can you attach a picture of your installs so I can have a look see if there is anything I could suggest...
I would but I already shipped it off. Trust me tho its not the install I made sure no wires are crossing and used thick wire for power a ground. I am guessing that I got a bad batch of chips.
are you all using your Duo3s with HD cables? use a standard AV cable and I bet PS1 games work and look fine.
I am using standard sony cables .. Anyways I no longer stock these.
