When the XO-2 still existed (or at least was marketed) I bought some to keep installing them...
I still had 2 of them, and installed one... and it worked fine for a night... At the next day PS2 backups didn't work... but PS1 still worked.
I re-checked everything and ir refused to work. Then I exhanged it for the other one and after some tidbits like cutting some long 30awg wires, it worked nice (because PS2 worked, but PS1 don't, something getting loose while exhanging the chip).
Now I've to install the last one (I'm out of stock) and the problem is the same... PS1 mode works, PS2 don't. If insert a PS2 backup and close the tray, RSOD (that's not normal, a working XO-2 never detects a disk until you do reset), but If I insert an original, it keeps trying to read the disk, as it was supposed to behave.
If I insert the backup and do a reset, it goes to the menu [Browser/System Config] for a second and then go to RSOD.
If I insernt an original and do a reset, it loads normally.
IF I insert an orignal PS1 game on PS2 side, RSOD.
PS1/Movie/Audio on PS1 mode works fine.
Any ideas on this behaviour? May be the chip fried?

I have the same problem.
Originally had the XO-2 installed in a v5 and was working fine, but later on only PS1 would boot and PS2 would red screen. I went ahead and installed it in a NEW v7 and it was the same situation.

to test a chip do this pwr gnd reset and scex
that will tell you if it works
