I have had this happen to me before when taking out the hard drive and copying images to it. I have also seen one other poster on this board and I think one on another report it as well.
It may be a wire touching something somewhere and causing the CC to boot its backup firmware as a safety precaution. I even had it happen once when BM was not even really running on this chip, just uLaunchElf which I renamed and substituted as the main BM elf file so after running the BM installer the console booted straight into uLauchElf 4.12 (I say this because someone told me that they were sure I erased it by accident in the BM menu).
So far I have never had this happen in such a manner that tapping reset three times would not allow me to reflash.
I said that I suspected that it may have been a connection touching something, but now that I think of it one time it did it when I never even moved the console. It was running fine, I was playing a game, then I turned it off as normal. When I powered it back up it came up to the Sony dashboard. It allowed me to reflash, but I did think it was kind of weird that it did that. It may be just been a glitch in the R34 firmware, because that is all that I have ever used on my chip. I think the other poster that had that problem was running R34 as well. For all we know CC may be aware of it and may have already fixed it.
I haven't had any problems lately, but I also don't play it that much anymore either now that I have finished GOW2. As a precaution whenever I copy new games over to it I usually do it over the network so things don't get moved around. This may help reduce the frequency of the problem.
